Notary Service Requirements for Travel Consent Forms
If your child is travelling outside Canada without both parents or legal guardians, you may be asked to provide a notarized travel consent form. This document confirms that the child has permission to travel and helps prevent delays at border crossings. To ensure your appointment goes smoothly, please review the requirements below and come prepared with the completed Government of Canada form and proper identification.

- Use the “Add a Name” and “Add a Child” buttons carefully:
- Each “Add a Name” or “Add a Child” adds another name field, and an entire contact information section, with address, phone, email, etc for each time you click add a name
- Review the number of people to be included before printing to ensure the form matches exactly those who need identification.
- Attend the appointment in person with valid photo ID that exactly matches the names on the travel form (passport, birth certificate, driver’s license, etc.).
- All individuals named on the form must attend the appointment.
For minors:
- If the minor is old enough to be reliably identified (typically school-age or older), they should attend in person with valid ID.
- For younger children who cannot attend, the parent or guardian must appear with valid ID and required documents. Please check with the notary if the minor’s physical presence is necessary.
